1.3Page 3, line 13, after the comma insert " including exhaustion of all rights of appeal

1.5Page 3, delete lines 17 to 25 and insert:
1.6"(1) a description of the data that were accessed or acquired; and
1.7(2) if disciplinary action was taken against an employee:
1.8(i) the number of individuals whose data was improperly accessed or acquired;
1.9(ii) the name of each employee determined responsible for the unauthorized access
1.10or acquisition; and
1.11(iii) the final disposition of the disciplinary action taken against the employee
1.12in response."
